ORGANIZATION OVERVIEW:  

Face to Face Health and Counseling is a non-profit organization dedicated to advancing economic and health equity for youth ages 11–24. We do this work through a comprehensive approach that includes housing, mental health, medical, youth justice, education & employment, and community services. We take a strengths-based and trauma-informed approach to our work and center the needs of young people impacted by racism and other forms of oppression and marginalization. Founded in 1972, Face to Face serves more than 3,000 youth annually.

JOB SUMMARY:

The Substance Use Navigator meets with unstably housed youth to provide education and training on an array of treatment interventions including harm reduction strategies, how to access care, and support in overcoming barriers to reduce risk of harm related to substance use. The Substance Use Navigator will provide education to at risk youth on healthy alternatives to substance use, support youth actively engaged in problematic substance use to increase knowledge of and assist in access to substance use treatment programs and provide ongoing support to those in recovery.

The position is full-time at 32 hours per week.

RESPONSIBILITIES:

  • Meets with unstably housed youth with SUD to discuss harm reduction, connect to resources, facilitate connections to treatment, and address barriers
  • Provide Narcan and education about how to administer to youth known to be using opioids
  • Provide education about safer use practices and safe use supplies
  • Create safety plans around relapse prevention, incorporating a Harm Reduction approach as needed
  • Utilize full range of substance use treatment options that best match the needs and preference of youth served
  • Provide training and education to Face to Face staff on role of Substance Use Navigator and how to refer to services
  • Provide education to youth about Substance Use Treatment options, how to access services and role of the Substance Use Navigator
  • Identify and support youth on their recovery journey using a person-centered approach
  • Maintain accurate records for data tracking and to measure outcomes
  • Monitor service outcomes and make adjustments to improve outcomes as needed
  • Attend SafeZone and program meetings as scheduled
  • Continue to stay abreast of emerging trends, best practices and changes in available resources
  • Facilitate tabling events with community partners at SafeZone
  • Build a network of community partners and resources for referrals
  • Facilitate support groups at SafeZone
  • Meet grant-specific productivity standards/target as set by department manager and supervisor.
  • Keep up to date with current changes in the field through the use of supervision, appropriate conferences, training, in-services and reading.
  • Provides all services guided by the evidence-based principles for helping youth overcome homelessness ; and
  • Perform other duties as assigned.

QUALIFICATIONS:

  • Licensed Alcohol and Drug Counselor (Preferred)
  • Dually Licensed as an LADC and Mental Health Professional in the state of MN (preferred)
  • First Aid and CPR certification including administration of Naloxone/Narcan.
  • Maintain licensure or certification and stay current on continuing education requirements.

Or:

Licensed Mental Health Professional (LPCC, LICSW, LMFT) with 1 year experience with chemical health and substance use disorders.

Or:

Bachelor's degree or higher in social services, psychology, addiction studies or related field with 2,000 hours of supervised experience in substance use disorder treatment.

Or:

Certified Peer Recovery specialist with 2,000 hours of experience of related experience.

  • Demonstrated ability to work and communicate effectively with people of diverse ethnic, economic,

racial and cultural backgrounds.

LOCATION:

Essential Onsite – This role requires the employee to be onsite.

PHYSICAL DEMANDS AND WORKING CONDITIONS:

  • Ability to perform the duties outlined in the job description.
  • Ability to respond to crisis situations and administer 1 st aid when appropriate.
  • Safely lift and carry objects weighing up to 50 pounds
  • Able to be flexible, work under stress and to respond to emergencies/crises
  • Valid driver's license, insurance, and ability to drive in all MN driving conditions
  • Face to Face is subject to the government mandate requiring employee COVID vaccination. All employees will be required to get vaccinated.

PAY & BENEFITS:

This position is 32 hours per week. The pay rate for this position is between $32-36 an hour, based on licensure and qualifications.
