Job Description

Title: Care Navigator (Social Care Needs Screener and Navigator)        

Salary: $20.00 per hour

Status: Temporary, Part-Time, Non-Exempt, 20 hours weekly (2 part-time positions available)

Supervisor: Director of Clinical Administration

Schedule: Hybrid with travel to Behavioral Health Outpatient Centers in Dutchess & Ulster Counties required

 

Function:

The Social Needs Screener and Navigator plays a critical role in supporting Family Services (FSI) clients with Medicaid to address their health-related social needs, through screening (identifying), assessment and navigation (linkage) to services. This position is primarily remote with the expectation to occasionally travel to FSI sites and to attend community events to engage the community in screening and navigation. Seeking part time applicants. 

 

Job Requirements:

  • At least one-year’s previous experience working directly with clients in a health and human services field.
  • Associate degree in health, social services, or related field preferred. Equivalent work experience in a related field may be considered in lieu of degree requirements.
  • Experience with documentation with an electronic health record system or other database.
  • Spanish speaking preferred.

  • Strong computer skills and ability to learn relevant technology platforms for data entry and documentation.

Job Responsibilities: 

  • Screening: Identifying Medicaid members' social care needs using standardized tools.
  • Eligibility Assessment: Determining individuals' eligibility for enhanced services offered through the Social Care Network (SCN) program.
  • Navigation: Connecting individuals to appropriate services, including enhanced SCN services (housing, nutrition, transportation, care management) or existing federal, state, and local programs.
  • Collaboration: Working with other FSI and SCN staff, partner organizations, and health care and service providers to ensure seamless service delivery.
  • Documentation and Reporting: Accurately recording screening results, eligibility assessments, referrals, and follow-up activities in the SCN data platform.
  • Outreach: Engaging with individuals to inform them about available services and encourage participation in the SCN program.
  • Advocacy: Advocating on behalf of clients to access necessary services and address barriers to care.
  • Education: Educating clients about available resources and assisting them in navigating healthcare and social service systems.
  • Assist with setting up and maintaining accuracy of FSI’s information in the reporting platform.
  • Continuous Improvement: Participating in training and providing feedback to improve SCN effectiveness.
  • Other duties as assigned.
